Ruth’s home care package funded a visiting dementia consultant to support her with her memory issues, understand her needs and what was important to her to remain independent at home. The dementia consultant also recommended an occupational therapist who could complete a home safety assessment. With her care manager’s help, Ruth chose which recommendations from the assessment would be most valuable and important to her. The care manager arranged for a date and time clock, a medication reminder alarm and a whiteboard for reminders of daily appointments. The care manager also coordinates regular phone contact with Ruth’s family and for neighbours to check-in. In-home care services staggered across the week also work to extend Ruth’s social connections.
