The dementia consultant also recommended an occupational therapist who could complete a home safety assessment. With her care manager’s help, Ruth chose which recommendations from the assessment would be most valuable and important to her. The care manager arranged for a date and time clock, a medication reminder alarm and a whiteboard for reminders of daily appointments. The care manager also coordinates regular phone contact with Ruth’s family and for neighbours to check-in. In-home care services staggered across the week also work to extend Ruth’s social connections.
